The Wyoming State Board of Pharmacy established a Prescription Drug Monitoring Program in July, 2004 as authorized in W.S. 35-7-1060. The Board collects Schedule II-V controlled substance prescription information from all reporting “dispensers” as required by law. Reporting dispensers are required to provide this data to the Board no later than the close of business on the business day immediately following the day the controlled substance was dispensed (approximately every 24 hours).
I understand that inappropriate access or disclosure of this information is a violation of Wyoming Law and may result in disciplinary action by my licensing board and/or revocation of database access privileges.
I understand that all registrations and licenses must be current for me, if applicable or my supervisor, in order to access the online WORx database. The registrations and licenses may include the following:
I certify that my supervisor or I have a pharmacist/patient or practitioner/patient relationship for the records that I am accessing. As referenced in WY Controlled Substances Act Rules Chapter 8.
By accepting the terms of this agreement I hereby agree not disclose nor misrepresent any data or protected health information to any unauthorized persons or party. I agree that I will not share WORx account information, or my login credentials, or passwords.
