The Board established a Prescription Drug Monitoring Program in July, 2004, as authorized in W.S. 35-7-1060. The Board collects Schedule II-IV controlled substance prescription information from all resident and non-resident retail pharmacies that dispense to residents of Wyoming.
I understand that inappropriate access or disclosure of this information is a violation of Wyoming Law and may result in disciplinary action by my licensing board and/or revocation of database access privileges.
I understand participation in the WORx program is voluntary.

I agree to not disclose nor misrepresent any data or protected health information to any unauthorized person or party. I agree that I will not share my account information, login name, or password with anyone.
